Collaborative Drug Therapy Management is an educational reference researched, written and published by Collab Cares. By using collaborativedrugtherapymanagement.com you agree to the terms below. They took effect on September 6, 2026.

Educational purpose

Everything on this site, including each state page, comparison table, glossary entry, cost estimate and weekly brief, is provided for general educational and informational purposes only. It sets out our reading of state statutes, administrative codes and licensing-board rules as they stood when we last reviewed them.

Nothing here is legal, regulatory, clinical or financial advice, and nothing here is a substitute for it. Reading this site, or building a document through it, does not create an attorney-client, advisory or any other professional relationship with Collaborative Drug Therapy Management or with Collab Cares.

Accuracy and review

Laws, rules and board guidance change often, and they change without notice to us. We review the state research behind this site on a near-weekly basis and record the review date on each state page. Even so, there can be a gap between a change taking effect and its appearance here, and any page may contain errors, omissions or material that is out of date.

Before you rely on anything on this site for a licensing, compliance, contracting or hiring decision, confirm the current requirement with the governing board and with a licensed attorney in your state. The citations on each page identify the primary sources to check.

Limitation of liability

To the fullest extent permitted by law, Collaborative Drug Therapy Management, Collab Cares, and their respective owners, officers, employees, contributors and affiliates are not liable for any loss, damage, penalty, fine, claim or expense arising out of, or connected with, your use of this site or your reliance on any summary, estimate, comparison, brief or document obtained through it. This exclusion covers direct, indirect, incidental, consequential, special and exemplary damages, and it applies even if we were told that such a loss was possible.

The site and its content are provided as is and as available, without warranty of any kind, express or implied. That includes any warranty of accuracy, completeness, timeliness, merchantability, fitness for a particular purpose and non-infringement. Where a jurisdiction does not allow one of these exclusions, our liability is limited to the smallest amount that jurisdiction permits.

Authorship and copyright

The research and editorial content of this site is authored by Collab Cares. Quotations from statutes, rules and board publications are reproduced from the public record and are cited where they appear. All other text, tables, comparisons and data feeds are the editorial work of Collab Cares and are protected by copyright.

You may link to any page and quote short passages with attribution to Collaborative Drug Therapy Management. Reproducing pages in full, republishing the data feeds or presenting this content as your own requires our written permission.

Documents built on this site

An agreement you build here is authored and delivered by Collab Cares as a starting draft. It is prepared from the answers you supply and from our understanding of your state's requirements at the time. It is a template, not legal advice, and it is not tailored to facts we have not been given.

Have a licensed attorney in your state review any agreement before it is signed, and check whether your board requires the agreement to be filed, registered or approved.

The builder is free, creates no account and asks for no payment details. To author your draft and send it to you it collects your name, email address and phone number; your practice's name, type and address; the name and degree of the collaborating or supervising physician; the name and role of each provider the agreement covers; the clinical scope you select; the agreement dates; and any monthly fee you enter. It collects no patient information and asks for none — do not enter any.

We email the draft to you and to nobody else. The physician and providers you name are named so the document can be written: we do not contact them, do not enrol them in anything and do not add them to any mailing list. We keep your submission against the email address you gave, so that if you later open a Collab Cares account with that same address your draft is waiting in it rather than lost. That is the only thing your address does here — it creates no account and subscribes you to nothing.

To have a submission deleted, whether you built it or you are named in it, write to team@collabcares.com. You do not need an account to ask. Your use of the builder is also governed by the terms and privacy policy published at collabcares.com.

Analytics and bot protection

This site uses Google Analytics 4 and Cloudflare Web Analytics to understand how it is used: which pages are read, how visitors arrive, and which features are used. Google Analytics sets cookies and processes your IP address and browser information under Google's privacy policy; we send it no name, email address or document contents. Cloudflare Web Analytics is cookieless and does not track you across sites. Links between Collaborative Drug Therapy Management, its sister sites and Collab Cares carry a source tag so we can see which site sent a visitor. You can decline Google Analytics with your browser's cookie controls or Google's opt-out browser add-on.

The agreement builder is protected by Cloudflare Turnstile, which evaluates browser signals to tell people from automated traffic and may show a brief challenge; Cloudflare processes those signals under its privacy policy. What you enter in the builder is sent to Collab Cares to author your document and is governed by its terms and privacy policy.

These sites are published for a United States audience and do not show a cookie-consent banner. Google Analytics is the only thing here that sets a cookie, it is disclosed above, and you can decline it with your browser's cookie controls or Google's opt-out browser add-on. The signed-in Collab Cares application at app.collabcares.com carries no Google Analytics at all.

Machine learning. We do not use the answers you give the agreement builder, or anything else you enter on these sites, to train machine-learning models, and we do not provide that content to anyone else to train theirs. We do use a language model to help assemble our clinical vocabulary — it is given one clinical term at a time, with its definition, and never a document, a name or a builder submission.

Changes to these terms

We may revise these terms from time to time. The version posted on this page is the version in effect, and the date at the top of the page is the date it took effect. Continued use of the site after a change means you accept the revised terms.

If a page is wrong or out of date, or if you have a question about these terms, contact Collab Cares through collabcares.com. Corrections are reviewed in the same near-weekly cycle as the rest of the research. You can return to the home page at any time.
